Abstract
缺血性中風仍是全球主要的死亡與長期失能原因,其治療困難常來自血腦屏障(BBB)難以穿透、治療時效狹窄,以及缺乏控制的無菌性發炎反應。本研究提出一種具震波響應性的奈米酶載體(pEV-GCS@Hb),外層包覆血小板衍生囊泡,旨在以協同式生物能量調控突破上述臨床瓶頸。該奈米載體整合甘醇幾丁聚醣(GCS)作為壓電刺激基質、血紅蛋白(Hb)作為氧氣/氫氣的氧化還原來源,以及血小板囊泡實現發炎標靶遞送。在低強度震波(SW)啟動下,平台可誘導暫時性 BBB 開啟、氣體傳導分子釋放,以及局部電訊號產生。動物實驗顯示,此級聯反應可在 24 小時內顯著降低腦水腫(約 70%)、促進小膠質細胞由 M1 向 M2 表型轉化,並恢復步頻參數(約 40%),且未觀察到全身毒性反應。本研究證實 pEV-GCS@Hb/SW 系統具備作為急性缺血性中風治療之非侵入式、可程式化與多模態奈米醫療策略的潛力。
