Abstract
研究背景:世界衛生組織推估全球焦慮症的病患中,3.8%為老年族群。當老年人感到焦慮時,經常不會積極處理,直到嚴重影響生活時才會就醫治療,若能事先篩選出有焦慮傾向的長者,就能及早介入,減少問題的發生。因目前適合評估老年人焦慮的工具仍不足且缺乏切分點,因此,本研究將翻譯Pachana等人針對長者所發展的「老人焦慮量表」 (Geriatric Anxiety Inventory, GAI),以提供學者或相關人員於評估老年焦慮時使用。
研究目的:本研究旨在建構台灣版老人焦慮量表的信度與效度。
研究方法:採橫斷式研究設計,先進行GAI量表翻譯,再使用方便取樣,於北部長青會及長青關懷站進行收案,收案條件為:(1)年齡65歲(含)以上、(2)意識清楚及(3)能閱讀或瞭解問卷內容者。排除條件為:(1)經醫師診斷有失智症、瞻妄及精神疾病與(2)嚴重視覺及聽覺障礙者。使用結構式問卷進行資料收集,包括:(1)基本資料表、(2)台灣版老人焦慮量表(Geriatric Anxiety Inventory-Taiwan, GAI-TW)及(3)中文版情境特質焦慮量表 (Stat-Trait Anxiety Inventory, STAI)。以SPSS18.0進行資料分析,使用次數分配、百分比、平均數及標準差呈現人口學及焦慮狀況,並以內容效度、建構效度、效標關聯效度、內在一致性信度及再測信度檢測量表之效度與信度。
研究結果:本研究共收案337位,研究結果顯示台灣版老人焦慮量表的Cronbach’s alpha為 0.93,再測信度之組內相關係數 (intraclass correlation coefficient, ICC)為 0.90。量表内容效度 (scale-level CVI, S-CVI)的普遍協議 (universal agreement,UA)為 1.0,S-CVI/Ave (average)為1.0。以探索性因素分析萃取出三個因素,分別為(1)認知焦慮 (cognition anxiety)、(2)焦慮衝擊 (impact of anxiety)及(3)身體焦慮 (somatic anxiety),可解釋變異量為59.46%。量表效標關聯效度檢測r = 0.62 (p < 0.001)。另外,台灣版老人焦慮量表在社區長者的最佳切分點為9/10。
結論及建議:本研究發現台灣版老人焦慮量表具有良好的信效度。此翻譯量表可提供相關人員評估長者的焦慮情形,以便及早發現問題,提供合適的照護。也可以提供學校教師或醫院主管參考,教導學生或醫護人員老人焦慮量表的使用及計分方式,以便應用於實習或工作中。另外,也可以提供未來老年焦慮相關研究使用。
