Abstract
背景:隨著人口老化,失智症患者將逐漸增加,有些研究指出心臟功能不佳可能會影響腦部血流,進而導致認知功能受損以及產生失智症。然而,尚未有研究探討過二尖瓣脫垂與失智症的關係。
目的:利用台灣涵蓋全國人口的資料庫來探討二尖瓣脫垂與失智症之間的相關性。
方法:本研究利用台灣的全民健康保險研究資料庫之2005年承保抽樣歸人檔進行分析,找出8531位失智症患者,並考量患者的社會經濟地位特性和共病,使用傾向分數配對出對照組共25,593位(1:3的比例),再使用羅吉斯回歸分析計算病例組與對照組間的勝算比 (odds ratio),以評估二尖瓣脫垂與失智症之間的相關性。
結果:所有樣本中,有563(1.65%)人患有二尖瓣脫垂,病例組中有170 (1.99%)人患有二尖瓣脫垂,對照組中有393 (1.54%)人患有二尖瓣脫垂。本研究中發現二尖瓣脫垂與失智症有相關,病例組相對於對照組,其調整後勝算比 (odds ratio)為1.29(95%信賴區間為 1.08〜1.55)。當樣本為男性族群,勝算比為1.53 (95%信賴區間為1.13~2.08),有達顯著相關。然而,樣本為女性族群時,則無顯著相關。
結論:二尖瓣脫垂與失智症之間兩者有顯著的相關性,失智症患者相較於無失智症者患有二尖瓣脫垂的勝算較高。然而,在女性族群中,兩者疾病則無顯著相關。
