Abstract
研究背景:自體免疫疾病患病率約占總人口數的 4%至 5%,目前尚無根治的方法。治療主要集中在減輕症狀、減緩疾病進展以及調節免疫反應,虹彩炎是一種自體免疫系統疾病,與多數全身性自體免疫疾病有相關性。薛格連氏症為第二大類全身性自體免疫疾病,雖有文獻探討虹彩炎與薛格連氏症候群之相關性,但都是個案報告且僅有少數幾篇,缺少大型流行病學研究,且無亞洲種族大型研究,兩者之相關性在過去研究中尚無法被證實。故本研究欲探討虹彩炎與薛格連氏症候群的相關性。
研究目的:虹彩炎和薛格連氏症候群的相關性探討。
研究方法:本研究使用 2010 年承保抽樣歸人檔 (LHID2010),從中隨機取200 萬人,以 2013 年 1 月 1 日起至 2019 年 12 月 31 日期間,於上述期間年齡≤ 20 歲診斷為虹彩炎為研究對象,而後使用傾向評分匹配法(Propensity Score Matching),利用多元邏輯斯迴歸分析 (Multiple logistic regression analysis)計算傾向值,將控制變項以 1:3 的方式,使用最鄰近匹配(Nearest neighbor matching, NNM)的選取方式進行分析,並擷取有罹患虹彩炎個案為病例組,無罹患虹彩炎個案為對照組。
研究結果:本研究利用回溯性研究進行數據分析,共取得 62,504 位樣本數。經卡方檢定顯示,虹彩炎患者和對照組之間薛格連氏症候群的盛行率存在統計學顯著差異(3.79% vs. 1.57%,P<0.001)。在多元羅吉斯迴歸分析中,與對照組相比,粗勝算比為 2.474 (95% CI:2.217-2.761),校正後勝算比為2.535 (95% CI:2.268-2.833)。此外,在男性樣本患者中,與對照組相比,虹彩炎患者中之前薛格連氏症候群調整後勝算比為 2.494 (95% CI=2.010~3.095)。
結論:這項研究揭示了虹彩炎和薛格連氏症候群之間的有顯著相關性。從臨床角度來看,這些發現提供給醫護人員,提升其對相關疾病的認識,並提倡對患者採取全面的與多科別的照護,避免患者面臨視力喪失此類不可逆的損傷。
