Abstract
背景:失眠與認知功能障礙是老年人腦中風後常見後遺症。然而,雖然過去研究確立了腦中風、失眠與認知功能的相關性,但失眠是否加重了腦中風老年人認知功能障礙的風險尚未清楚。
目的:探討老年人腦中風後失眠與認知功能之相關性。
方法:本研究採用次級資料庫分析,利用2009年「國民健康訪問調查」之數據,納入65歲以上主訴曾經腦中風並主述有失眠以及曾腦中風但未有失眠之受測者,並以簡短智能測驗做為認知障礙的依據。採用SPSS 18.0統計軟體,以描述性統計計算中風後失眠之盛行率,並使用多元邏輯斯迴歸探討有無失眠之腦中風病人與認知功能間的相關性,統計顯著性p設定為< 0.05。
結果:本研究共收案447位老年人,其中,149位為主訴曾經中風之老年人,以性別與年齡進行配對後,隨機選樣298位未有中風主訴之老年人。平均年齡為74.63±6.44歲。中風後失眠盛行率為38.9%,中風後出現認知障礙盛行率為50.3%。本研究結果發現,在控制潛在危險因子後,中風合併失眠組以及有中風無失眠組罹患認知功能障礙的勝算比分別為4.45與2.74(95% confidence interval =2.01-9.85以及1.45-5.17),皆達統計上之顯著差異。亦即腦中風後有較高勝算比發生認知功能障礙,但若腦中風後出現失眠,會加重發生認知功能障礙可能。
結論:腦中風後失眠有更高的機率有認知功能障礙。針對腦中風後失眠的病人,醫護人員應規劃非藥物療法來治療其失眠問題,以降低認知功能障礙的機率,並應用於臨床常規照護評估之中。
