Abstract
研究背景
順鉑和卡鉑是廣泛使用的鉑金類化療藥物,但其臨床應用常受限於腎毒性。儘管已有許多研究利用機器學習預測鉑金類藥物引發的腎毒性,但大多數模型依賴回顧性數據,且缺乏前瞻性驗證,限制了其臨床適用性。
研究目的
本研究旨在前瞻性驗證兩個先前建立的機器學習模型,用於預測多個化療週期中鉑金類藥物引發的急性腎損傷 (AKI) 和急性腎病 (AKD) ,並將其表現與臨床醫師的評估進行比較。
研究方法
這項前瞻性單中心世代研究於 2023 年 10 月至 2025 年 8 月期間,納入了接受順鉑或卡鉑治療的癌症患者。本研究驗證了兩個機器學習模型,分別用於預測每個化療週期後 14 天內的 AKI 和 89 天內的 AKD。模型輸入數據包括基本資料、共病症、化療資訊、腎毒性藥物的使用以及實驗室數據。模型預測結果與透過五點量表收集的臨床醫師評估進行比較。模型表現主要使用受試者操作特徵曲線下面積 (AUROC) 和精確度-召回率曲線下面積 (AUPRC) 進行評估。本研究調整閾值以確定最佳閾值,並進行了存活分析以及跨次族群的公平性評估。特徵重要性則使用SHAP (Shapley Additive exPlanations) 進行解釋。
研究結果
本研究共納入 77 名患者,總計 361 個化療週期,其中 19 名患者發生 AKI,34 名患者發生 AKD。機器學習模型展現優於臨床醫師的表現,AKI 模型的 AUROC 為 0.801,AUPRC 為 0.313;AKD 模型的 AUROC 為 0.705,AUPRC 為 0.586。經存活分析驗證及跨次族群的公平性評估後,為優化風險分層,最佳閾值修正為 0.62 (AKI) 和 0.43 (AKD)。經驗證的模型已整合至網頁應用程式中。
結論
這些經過前瞻性驗證的模型可作為預測鉑金類藥物相關腎毒性的監測工具,能在每個週期實現早期風險評估與個人化患者管理。
