Abstract
研究背景
使用呼吸器病患約有5%到13%發展為長期使用呼吸器病患,平均每年住院使用呼吸器病患超過一萬六千人次,其中呼吸器使用超過21天者約占17%,超過63天者約占6%,使用天數超過121天者約3.8%。2012年呼吸器患者全年的醫療費用點數達168.81億點,占全國醫療費用2.98%,全年平均每人醫療費用僅次於血友病,每一呼吸器患者約為國人平均費用之29.2倍,呼吸器患者的醫藥費已成為健保財務的沉重負擔。呼吸器治療過程因插管及機械性通氣,而增加了肺炎發生率,氣管插管的病患發生率約為8~28%,比未使用呼吸器的病患高出6至21倍。文獻指出,在台灣呼吸器相關肺炎占住院肺炎生的52%,死亡率為43.48% (姚俐音、王玠仁、張承圭、蔡慧琇、陳佳慧,2008)。本研究探討影響呼吸器相關肺炎發生之相關因素,以作為未來呼吸器依賴患者照護品質改善政策之建議。
研究目的
本研究以全民健保資料庫百萬歸人檔為研究資料來源,探討長期使用呼吸器病患發生呼吸器相關肺炎之相關因素,具體而言,本研究之研究目的為:
一、 探討台灣地區人工氣道是否因病人特質或醫院特質而有差異。
二、 探討台灣地區呼吸器依賴患者呼吸器相關肺炎之發生是否因人工氣道而有差異。
三、 探討台灣地區呼吸器依賴患者呼吸器相關肺炎之發生是否因病人特質而有差異。
四、 探討台灣地區呼吸器依賴患者呼吸器相關肺炎之發生是否因醫院特質而有差異。
研究方法
本研究利用全民健保資料庫百萬歸人檔之「醫事機構基本資料檔」(HOSB)、「住院醫療費用清單明細檔」(DD)、「住院醫療費用醫令清單明細檔」(DO),針對長期呼吸器使用呼吸器患者採用複迴歸分析,以了解人工氣道、病人特質及醫院特質對呼吸器相關肺炎發生之影響。
研究結果
研究對象共計4,280人,病患特質如性別、年齡、呼吸衰竭相關主診斷及共病指標與呼吸器相關肺炎發生風險無顯著差異,相較於使用氣管內管的病患,使用氣切套管的病患發生呼吸器相關肺炎的風險較低,具統計上顯著差異(P<0.001),使用氣切套管約為氣管內管的0.46倍。醫院層級愈高,發生呼吸器相關肺炎風險愈高,呈正相關;權屬別以私立醫院發生呼吸器相關肺炎風最低,具統計上顯著差異(95% CI:0.201,0.857)。
結論
研究結果顯示,相較於使用氣管內管的病患,使用氣切套管的長期呼吸器依賴(使用超過30天)的病患,一年內發生住院肺炎的風險顯著(P<0.001)較低,約為0.466倍。鼓勵選擇氣切套管使用,落實院內呼吸器相關肺炎感染防治,以降低呼吸器相關肺炎發生風險。
