Abstract
慢性阻塞性肺病是一種呼吸氣流受持續性限制的呼吸道症狀。目前已知吸菸和空氣汙染是影響慢性阻塞性肺病的主要危險因子,造成多樣化的疾病表徵且其發病機轉不明,也無有效的治療策略。開發可用於分群慢性阻塞性肺病表型的生物標誌將有助於探索可能的治療策略。在這篇論文中,我們建立血清貹肽圖譜庫與數據非依賴性採集質譜(DIA-MS)平台,並將其應用於深入分析慢性阻塞性肺病的患者的血清蛋白體。首先,我們使用多種血清混合樣品進行高含量蛋白去除和多種貹肽或蛋白質分離方法,並以分段掃描分離模式 DIA-MS建立完整的血清貹肽DIA圖譜庫。此外,我們也優化資料處理和分析流程,以提升蛋白質鑑定和定量分析之準確性。我們將優化後的質譜平台應用於個別分析 81 位慢性阻塞性肺病患者血清樣本,總共鑑定952蛋白質,並成功定量592個蛋白質。根據病患血清蛋白體的表現歸類出四群分子亞型病人,並篩選出 8 個蛋白作為病人分群之生物標記組合。我們也找到與晚期患者、經常惡化、急性惡化、肺功能快速衰退、肺氣腫、高嗜酸性粒細胞與吸菸患者表型中的差異表達蛋白並探討其在慢性阻塞性肺病之角色。值得注意的是,我們總共發現了 23 個參與超過兩種以上表型的重要蛋白質,這些蛋白質可作為各項 COPD 表徵型診斷的標記。我們認為此篇研究幫助了解各項不同臨床表徵慢性阻塞性肺病的獨特血清分子表現,將有助於未來在臨床上辨識不同病理機制之慢性阻塞性肺病。
