Abstract
研究動機與目的
吸入口咽或胃內容物可能引發急性肺損傷,其病理機制包括上皮屏障功能受損、顯著炎症反應、氧化壓力增加,以及細胞凋亡等多重途徑。目前臨床治療方式大多偏向支持性照護,尚無可同時調控多種病理途徑的有效治療策略,因此急需發展具多重調節能力的新型治療方法。間質幹細胞衍生的外泌體能攜帶具有生物調控作用的微小RNA,被視為具有潛力的細胞外治療工具。其中,hsa-let-7i-5p在胎盤來源間質幹細胞之外泌體中表現豐富,但其在吸入性急性肺損傷中的生物學功能與作用機制仍未完全釐清。本研究旨在評估間質幹細胞衍生之外泌體對吸入性急性肺損傷的治療效果,並進一步探討hsa-let-7i-5p在其中所扮演的分子調控角色。
研究方法
本研究以口咽灌注方式建立小鼠吸入性急性肺損傷模型,並自人類胎盤分離與鑑定間質幹細胞衍生之外泌體。於肺損傷發生後,透過氣管內給藥方式施行外泌體治療。研究評估項目包括肺功能測試、組織病理分析、支氣管肺泡灌洗液細胞組成、肺水腫指標,以及發炎蛋白、氧化壓力與細胞凋亡相關指標。
此外,本研究結合蛋白質體學與生物資訊分析,以描繪吸入性肺損傷相關的分子變化,並分別使用hsa-let-7i-5p抑制之外泌體(功能喪失模型)及富含 hsa-let-7i-5p 的工程化外泌體(功能替代模型),以驗證該微小RNA在治療效果中的必要性與充分性。
研究結果
研究結果顯示,吸入口咽或胃內容物會造成典型急性肺損傷表現,包括肺順應性下降、氣道阻力增加、肺組織結構破壞、中性球浸潤、氧化壓力上升及細胞凋亡增加。間質幹細胞衍生之外泌體可顯著改善上述損傷指標,並協調性地調控發炎反應、氧化壓力與細胞死亡相關路徑。當hsa-let-7i-5p功能受到抑制後,外泌體的治療效果明顯減弱;相對地,富含hsa-let-7i-5p的工程化外泌體在提高粒子劑量後,可於相同實驗條件下重現多項關鍵保護效應。
結論
綜合而言,間質幹細胞衍生之外泌體能有效改善吸入性急性肺損傷,並透過多重病理途徑的協同調節發揮治療效果。其中,hsa-let-7i-5p為外泌體治療作用中不可或缺的關鍵調控分子。本研究深化了外泌體治療吸入性肺損傷之分子機制理解,並支持工程化外泌體作為功能驗證與機制釐清工具,為未來相關轉譯研究提供重要的實驗基礎。
