Abstract
創傷性腦部損傷(Traumatic brain injury;TBI)世界各國都一直是個值得注意的公共衛生議題,全球每年約有140 萬人受到創傷性腦部損傷,而隨著盛行率的逐年增加,不得不重視這個日益嚴重的問題。過去許多文獻皆指出在發生輕度創傷性腦部損傷後(mTBI),憂鬱症和焦慮症都是情感性疾病中最常見的診斷;然而目前對於這些輕度創傷性腦部損傷後引起情感性疾病的預測方式依然不甚了解。根據研究闡明,憂鬱症和焦慮症皆與自主神經調控的心率變異性(HRV)之改變息息相關。心率變異的頻譜分析廣泛運用於作為一種測量自主神經系統(ANS)功能的指標。因此我們藉由研究輕度創傷性腦部損傷病人心率變異的功率頻譜之分析,研究是否用於預測輕度創傷性腦部損傷病人日後是否會發生情感障礙,特別是焦慮症以及憂鬱症。
研究對象包含輕度創傷性腦部損傷病人,以及健康的自願受試者。並且透過填寫貝氏焦慮量表(BAI)以及貝氏憂鬱量表(BDI)的方式,依據其測量分數分別將創傷性腦部損傷病人和健康受試者分成有焦慮卅憂鬱症狀或無的組別,並且根據這些組別進行個人的五分鐘心率變異測量。持續追蹤第六週及第十二週,我們發現在創傷性腦部損傷病人以及健康受試者的貝式焦慮分數、貝式憂鬱分數以及心率變異的各項參數之間與疾病有顯著相關性。雖然這些初步的結果的收案樣本數較小,但它依然可以提供一些支持性的方向---藉由參考心率變異的變化,作為預測輕度創傷性腦部損傷病人,日後是否比健康者較容易發生情感性疾病,例如焦慮症或憂鬱症,而有助於臨床早期診斷與後續治療。
