Abstract
步態的型態在頸椎椎管狹窄疾患中是一項重要的徵象。特別是在早期階段罹病的頸椎椎管狹窄患者身上是非常難以診斷的。在神經外科中,步態可以視為是神經和肌肉相互協調下產生的動作。然而,目前的醫學診斷方式,醫生主要是使用Nurick grade量表此主觀的方式來判定,但因不同醫生的主觀判定而有所不同。
步態分析可以用來評估及測量人類走路的行為模式。近年來大多數相關研究專注於將步態資訊量化,建立客觀的測量步態方法是非常重要的。本研究的目的是比較Nurick量表等級和頸椎椎管狹窄患者的T2加權核磁共振成像(MRI)的步態分析,並確定的步態參數的Entropy。我們希望有一天,三軸加速器將能夠使用於預測跌倒的發生。
為了確定Entropy在頸椎椎管狹窄患者之步態和Nurick grade量表之關聯性。本實驗病人的評估方式將透過Nurick grade, 頸椎核磁共振和步態分析來進行。步態分析的數據信號是透過三軸加速器來收集,收集步態進行中垂直軸之entropy的頻率和entropy的振幅。本論文中頸椎椎管狹窄的個案挑選,是由三位神經外科醫師判斷,判斷患者頸椎核磁共振T2顯影下信號強度是否增加及Nurick grade級數。
實驗組為罹患頸椎椎管狹窄患者併頸椎核磁共振T2顯影下信號強度增加者,對照組為頸椎椎管狹窄患者但頸椎核磁共振T2顯影下信號強度無增加者。比較後發現頸椎椎管狹窄患者併頸椎核磁共振T2顯影下信號強度增加組別,於步態進行中的站立期和擺盪期的垂直軸之entropy的頻率較對照組的entropy高並俱有顯著差異,分別為站立期 (P=0.0115) 和擺盪期 (P=0.0007)。此外,在於Nurick grade級數方面,兩組並無顯著差異。
我們發現entropy在手術前比手術後高,於步態進行中的站立期和擺盪期皆俱有顯著差異 (P=0.0313)。最後,我們發現Nurick grade於手術前和手術後並無顯著差異(P=0.5000)。
我們發現嚴重的步態障礙可能entropy的頻率較高。在未來,頸椎椎管狹窄患者的步態評估可以使用entropy的頻率來分析較Nurick grade分級客觀,可提早預防患者跌倒的發生,進而達到傷害防治的目的。
