Abstract
腹膜透析(PD)為被廣泛應用腎臟替代療法。然而,長期使用高糖腹膜透析液可能導致腹膜反覆受損,進而引發腹膜纖維化,這會影響治療效果,最終導致超過濾失敗。壓力會誘發不同器官(包括肺、肝和腎上皮)的纖維化,但是壓力對腹膜的影響尚未被闡明。在本研究中我們提出:增加的壓力可能是腹膜纖維化的關鍵因素,並探討了可能的機轉。細胞實驗發現,壓力導致人類腹膜細胞MeT-5A呈現間質細胞表型,增加纖維化標記和炎症因子的表現。此外,壓力增強了MeT-5A細胞的增殖能力和遷移能力。來自腹膜透析小鼠PD模式和人體腹膜平衡試驗(PET)的數據都顯示,更高的壓力與增加的小分子溶質運輸以及超過濾量的減少有正向關聯。在機制層面,我們觀察到壓力下腹膜細胞中CD44表現亮顯著增加。有趣的是,使用CD44中和抗體可以抑制壓力誘導的腹膜細胞表型變化。而褐藻醣膠(Oligo-fucoidan)作為一CD44抑制劑,可以緩解腹膜透析小鼠中壓力誘導的腹膜增厚、纖維化和炎症現象。我們還研究了褐藻醣膠對透析液中高糖引起的腹膜損傷的潛在益處。高糖PD溶液(Dianeal 4.25%,Baxter)增加了MeT-5A細胞中間皮-間質轉化(MMT)的標記(如N-鈣粘蛋白、α-SMA和纖維連接蛋白)的蛋白表現,同時降低了過氧化氫酶的表達並刺激了氧化自由基(ROS)的產生。此外,我們在腹膜透析小鼠模式中觀察到巨噬細胞的湧入和血清中促炎細胞因子(如IL-1β、MCP-1和TNF-α)的增加。有趣的是,我們發現褐藻醣膠,通過其抗氧化效應、降低MMT標記蛋白的表現以及減少腹膜和全身炎症,有效地防止了腹膜透析相關的腹膜增厚和纖維化。因此,褐藻醣膠有可能被開發成為腹膜透析相關腹膜纖維化的新型預防藥物。
