Abstract
國內外少有對於老年人脫離呼吸器相關研究,阻礙老年人脫離呼
吸器的因素相對於一般成年人複雜且難以評估,因此面對老年人脫離
呼吸器的過程時更需要有個別化的照護和評估計畫。
本研究目的在探討老年人脫離呼吸器之客觀因素含病患基本特 質、疾病特質、生理指標。使用回溯性病歷回顧研究方法,以北部某 一醫院加護單位使用呼吸器且年齡大於 65 歲老年病患為研究對象, 研究結果得以下結論:老年人之年齡、插管天數經過統計分析呈現與 脫離呼吸器成敗有顯著意義,生理指標中每分鐘呼吸次數;淺快呼吸 指數和最大吸氣壓力,以上變項與脫離呼吸器成敗在統計上有顯著意 義。經邏輯斯迴歸分析後,插管天數、呼吸相關疾病史、心血管疾病 史、每分鐘呼吸次數和淺快呼吸指數為與老年病患脫離呼吸器成敗具顯著預測力的變項。因此,本研究建議在脫離呼吸器的過程,越早鑑 定病患自發性呼吸的能力、鑑別病患有無呼吸相關和心血管疾病史, 則可協助病患於早期脫離呼吸器,降低病患與家屬的負擔,節省醫療、 照護人力成本,讓醫療資源更有效利用。
