Abstract
本研究主旨在了解正位心臟移植病患與健康成人心率變異度之差異及探討正位心臟移植病患身體活動度與心率變異度之相關。本研究為一描述性、相關性的横斷性研究,收集接受正位心臟移植手術後一年之個案及與其年齡、性別與身體活動度相配對之健康成人為對照,以比較兩組個案心率變異度之差異。使用七日身體活動回憶量表調查研究對象在過去一週的身體活動程度,以探討身體活動度與心率變異度的相關性。共收集正位心臟移植病患60名,健康成人60名。研究資料分析以次數、百分比、平均值、標準差呈現描述性統計,再以t-test、Chi-square、Pearson correlation 進行檢定。
結果發現心臟移植患者幾乎各個心率變異度之參數值包括mean、 sdr、TP(ms2)、LF(ms2)、LF(nu)、HF(ms2)、LF/HF數值均小於健康成人,並達統計上之顯著差異(p <.05),心臟移植病患之心率變異度中,TP(ms2)、LF(ms2)、HF(ms2)及LF/HF之變異度僅為健康人之17.8%,2%,7.3%和44%。可見心臟移植患者歷經手術去神經化過程後的心率變異度顯著低於健康成人。心臟移植患者週相對中度以上身體活動量與sdr(r=.409, p=.015)、TP(ms2)(r=.428, p=.01)、HF(ms2)(r=.422, p=.012)呈顯著正相關,卻與LF(nu)呈負相關(r=-.337, p=.048);同樣的週絕對中度以上身體活動量亦與sdr(r=.424, p=.011)、TP(ms2)(r=.453, p=.006)與HF(ms2)(r=.466, p=.005)呈顯著正相關。研究結果顯示中度以上身體活動量愈大心率變異度愈佳。
本研究結果可提供護理人員對於心臟移植患者照護之參考,以提升心臟移植病患照護之品質。
