Abstract
肝癌的盛行率在台灣地區相當高,在眾多致癌因子中以B型肝炎病毒感染所誘發的肝癌人數為最多,此外經由B型肝炎所誘發肝癌的患者當中有較高的術後復發機率,已有研究指出,B肝病毒所導致之肝癌會以表觀遺傳學中DNA甲基化修飾調控癌細胞之癌幹細胞能力,而目前臨床醫療研究將針對有效抑制與不良預後相關的癌幹細胞調節機轉為主要目標。因此,本篇研究將探討HBV帶原之肝癌病患其介白素-6 (IL-6) 與幹細胞多能性因子OCT-4之表觀遺傳調控關係。首先我們從臨床肝癌病患檢體中發現IL-6與患者術後存活率,復發時間皆有相關性。同時,我們也發現肝炎病患腫瘤組織之OCT-4的基因表現量與其血清中IL-6濃度之間呈現正相關性,其中HBV帶原之病患之正相關性更為明顯。我們實驗室先前的研究亦證實,IL-6是HBV誘導多能性幹細胞基因OCT4 表現之重要因子,因此我們證明HBV所導致之肝癌是因協同IL-6作用與DNMT1和DNMT3b促使OCT4 啟動子開啟而使肝癌細胞表現多能性因子OCT-4,進而導致肝癌早期復發與不良預後發生。
