Abstract
面對高齡化、少子化、家庭照顧功能式微,整體人口長照需求提升,個人與家庭照顧壓力負擔繁重,為因應其對社會、經濟的衝擊,我國政府擬積極推動長期照顧服務及規劃長期照顧保險政策,而社區老人為長期照顧利用及長照保險服務的主要族群,其需求及態度值得進一步探討。本研究旨在探討社區老人「長期照顧需求」和「長照保險態度」現況、相關影響因素與其預測因子,並比較獨居與非獨居老人之差異。
本研究為橫斷式研究,使用自擬「社區老人長期照顧需求與長照保險態度」問卷,採立意取樣、面對面調查法收集資料,獨居老人及非獨居老人分別115人為研究對象,所得資料以SPSS for Windows 19.0進行統計分析。
結果發現:(1)獨居老人在各項長照需求程度及長照服務利用率皆高於非獨居老人,其中以「老人餐飲服務」的需求與利用率最高;非獨居老人較獨居老人傾向支持政府規劃長照保險制度及願意給付較高長照保險金額。(2)獨居、無商業保險、輕度心理困擾、ADL「中重度依賴」及IADL有失能者,其長照需求高;長照需求的預測因子為IADL失能項目數、是否為獨居。(3)獨居、無商業保險、有慢性疾病、有身心障礙、有使用輔具、ADL「中重度依賴」及IADL有失能者,有較高的長照服務利用;長照服務利用的預測因子為IADL失能數、有身心障礙及是否為獨居。(4)86.6%的研究對象贊成政府規劃長照保險,非獨居、有照顧者、有商業保險、自覺健康情形「佳」、無慢性疾病者,對政府規劃長照保險制度的態度較傾向贊成;對政府規劃長期照顧保險態度的預測因子有商業保險數、經濟收入及慢性疾病數。(5)研究對象願意支付長照保險保費金額平均為553元,非獨居、教育程度「大專以上」、經濟收入「4萬以上」、有商業保險、無慢性疾病、自覺健康情形「佳」者,願意支付較高的保險金額;願意支付長照保險金額的預測因子為經濟收入、教育程度、商業保險數及慢性疾病數。
根據本研究結果,建議(1)執行全面性獨居老人長照需求評估及擴充老人餐飲服務;(2)強化長期照顧服務、長保政策推廣,改善長照資訊不足現況;(3)定位健康服務中心角色,整備長照服務資源,俾利長照服務及長保政策能提供更適切、符合社區老人需求之服務。
