Abstract
研究背景與目的
失智症是一種漸進且不可逆的認知功能障礙,導致患者逐漸喪失日常功能和獨立性。隨著人口老化和失智症盛行率的增加,疾病的負擔也將顯著增加,使得失智症成為日益嚴重的全球公共衛生問題。目前,失智症的藥物治療選擇有限,在醫療領域中仍是一個很大的挑戰,因此有效的預防和管理措施是迫切需要的。多篇系統性回顧和統合分析發現,第二型糖尿病與失智症的風險增加有關,並強調了潛在的共同病理機制。而新型降血糖藥物Glucagon-like peptide-1 Receptor Agonist(GLP-1 RAs)的藥理作用除了調節血糖及體重外,在一些研究中也顯示出潛在的神經保護效果。然而,現有文獻對於GLP-1 RAs對失智症風險影響的證據有限且結果不一致,目前尚不清楚此藥物之神經保護效果是否能反映在失智症發病率的下降。因此,本研究旨在探討GLP-1 RAs的使用對第二型糖尿病患者罹患失智症風險的影響。
研究方法
本研究為回溯性的世代研究,利用台灣國家健康保險研究資料庫的兩百萬人抽樣檔進行分析,研究期間為2000年1月1日至2019年12月31日。研究共納入3,121名年齡45歲以上新診斷的第二型糖尿病患者。根據藥品處方記錄,將患者分為使用GLP-1 RAs用藥組和非用藥組。採用新使用者設計以減少偏差,新使用者定義為在研究期間首次使用GLP-1 RAs的患者。兩組分別以年齡、性別及干擾因子進行1:4的傾向得分匹配,以減輕混雜偏差,確保兩組之間的可比性。主要結果為失智症的發生率,包括全因性失智症、阿茲海默症、血管性失智症和其他類型失智症。研究使用Cox比例風險回歸模型分析兩組間失智症的風險比,同時依據累積用藥劑量、累積用藥日數及性別進行失智症風險的次族群分析,並進行敏感性分析以評估研究結果的穩健性。
研究結果
在本研究中納入的3,121名患者中,GLP-1 RAs用藥組相較於非用藥組顯著降低了全因失智症風險(aHR=0.561; 95% CI [0.330-0.954]; p=0.0327)。然而,對於阿茲海默症及血管型失智症的風險,兩組間並無顯著差異,而在其他類型失智症中,GLP-1 RAs用藥組顯示出顯著差異(aHR=0.543; 95% CI [0.309-0.955]; p=0.0339)。次族群分析發現,GLP-1 RAs在女性中具有顯著的保護效果(aHR=0.415; 95% CI [0.195-0.885]; p=0.0228)。儘管較低劑量和較短使用時間的組別顯示出降低失智症風險的趨勢,但未觀察到顯著的劑量或時間反應關係。敏感性分析驗證了結果的一致性與穩健性。
結論
研究結果表明,GLP-1 RAs的使用顯著降低了第二型糖尿病患者罹患失智症的風險,尤其是在女性族群中。儘管未觀察到明確的劑量或時間反應關係,但本研究為GLP-1 RAs的神經保護作用提供了有價值的見解。未來仍需進行更大規模的研究,以探索此神經保護效應的潛在機制。
