Abstract
本研究的目的是研究皮膚經投與點陣雷射處理後,檢視 5-aminolevulinic acid (ALA) 在體外試驗及活體試驗之經皮吸收的速率。點陣雷射運用一個創新的概念來增進藥物經皮傳遞的效率,此一創新的概念稱為 ”分數光熱解”。分數光熱解 (Fractional photothermolysis) 是指將單發的雷射光束能量分散成陣列式的微米雷射光點 (Microspots),不將表皮整片的氣化剝離,而是在立體結構上形成散落的的表皮柱狀氣化剝離,此角質層上的微米大小的柱狀氣化剝離就稱之為Microscopic treatment zones (MTZ)。MTZ有利於表皮移行再生。表皮移行再生的速度可用經皮水分散失的程度來進行評估。點陣雷射處理後的皮膚其表皮移行再生可在一天內修護完成。對照傳統雷射處理後的皮膚會有較嚴重的皮膚干擾及長達二天的皮膚修復期。實驗方面是採用直立式擴散裝置作為體外穿透試驗來加以測量 5-aminolevulinic acid 之經皮穿透速率。5-aminolevulinic acid 的經皮穿透率是依據各種雷射的能量 (2 J/cm2 和 3 J/cm2) 以及累積發數 (約 1 - 6 發) 來做函數的變化。經點陣雷射處理後的裸鼠皮其經皮穿透效果比未經處理之皮膚的穿透速率高達 27 - 124 倍的促進效果。而經點陣雷射處理後的豬皮其經皮穿透效果比未經處理之皮膚的穿透速率高出 3 - 260 倍的促進效果。另外,也採用過度增生的裸鼠皮作為穿透的屏障,此皮膚是以膠帶反覆撕除的方式刺激皮膚,使過度剝離的皮膚出現表皮增生之情況,結果發現 5-aminolevulinic acid 對於增生皮膚的穿透效果優於比未經處理的裸鼠皮。雷射共軛焦顯微鏡觀察活體在投與 5-aminolevulinic acid 後,皮膚內 Protoporphyrin IX (PpIX) 的分佈位置,如所預期的,在經過點陣雷射處理後,裸鼠皮的表皮下層及真皮上層有出現較強的紅色螢光。穿透深度也因點陣雷射處理後而增加。本研究證實低能量點陣雷射對增進 5-aminolevulinic acid 的經皮穿透能力是具安全且有效率的。
