Abstract
根據本實驗室先前研究結果得知,以魚油取代酒精液體飼料中25% 與57% 之橄欖油可以改善大鼠酒精性肝損傷,但魚油對於腦部的作用尚未釐清。因此,本研究聚焦於脂質代謝與胰島素抗性,探討魚油是否可以改善腦部損傷,並以肝-腦軸之觀點,探討其機轉與肝臟損傷之相關性。本實驗使用雄性 Wistar大鼠 (n=36) 分成6組,分別為控制組(Control, C)、魚油取代25% 橄欖油控制組(Control with 25% fish oil substitution, CF25)、魚油取代57% 橄欖油控制組(Control with 57% fish oil substitution, CF57)、酒精組(Ethanol, E)、魚油取代25% 橄欖油酒精組(Ethanol-containing with 25% fish oil substitution, EF25)、魚油取代 57% 橄欖油酒精組(Ethanol-containing with 57% fish oil substitution, EF57)等6組,分別給予調整過之Lieber-DeCarli液體飼料。飼養8週後犧牲,取肝臟、血液及大腦以供分析。與C組相比,E組病理切片脂肪變性與細胞浸潤評分、肝功能指標AST與ALT活性、氧化壓力指標cytochrome p450 2E1蛋白質表現量皆顯著上升。另外,與C組相比,E組的空間記憶顯著降低,且顯著增加腦部病理切片呈現細胞核萎縮現象及海馬迴IRS-1與下視丘GSK3β表現量。脂肪酸代謝路徑方面,與C組相比,E組增加肝臟三酸甘油酯 (Triglycerides, TG) 濃度、肝臟adipoR2與p-AMPKα蛋白質表現量顯著降低,且肝臟與腦部毒性脂質ceramide含量顯著上升。在胰島素代謝路徑方面,與C組相比,E組肝臟胰島素訊號路徑IRS-1、p-akt、p-akt/akt ratio蛋白質表現量、血液中葡萄糖及IGF-1濃度皆顯著降低,而海馬迴GLUT4與IGF-1蛋白質表現量顯著降低。而與E組相比,魚油取代部分橄欖油後,顯著改善酒精性肝臟與腦部損傷,包括:改善脂肪變性與發炎細胞浸潤、降低血漿總膽固醇 (Total cholesterol, TC)、TG、低密度脂蛋白 (Low-density lipoprotein, LDL-C)、TC/HDL-C比值及肝臟TG濃度。此外,在EF57組顯著增加血漿adiponectin濃度、肝臟ACO-1與CPT-1 mRNA含量,並且降低海馬迴IRS-1與下視丘GSK3β蛋白質表現量。由此可知,慢性酒精攝取會造成肝臟損傷、腦部損傷、肝臟與腦部之脂肪酸代謝異常和之胰島素阻抗,而在魚油取代部分橄欖油後,魚油具有直接抑制酒精誘導之腦部IRS-1與GSK3β上升之作用。
