Abstract
研究背景:慢性腎臟病(Chronic Kidney Disease, 以下簡稱CKD)為全球重要的公共衛生議題,隨著人口老化與慢性疾病盛行,CKD的發生率與疾病負擔逐年上升。腎功能惡化會增加心血管疾病風險,並加速患者進入末期腎病階段,需接受透析治療,對個人健康與社會醫療體系均造成重大衝擊。近年來營養狀況與慢性腎臟病進展之間的關聯性逐漸受到重視,血清白蛋白、血磷與血紅素等生化指標被認為可能與腎功能惡化密切相關,但相關實證資料仍不足。
研究目的:本研究旨在探討CKD患者營養指標與腎功能變化之間的關聯性,評估血清白蛋白、血磷及血紅素等營養生化指標是否可作為預測腎功能惡化之重要參考,並藉此為未來臨床營養介入及慢性腎臟病管理策略提供實證基礎。
研究方法:本研究採用回溯性設計,資料來源為2019年至2024年間某醫學中心之709位CKD患者之臨床資料,分析指標包括營養指標(血清白蛋白、血紅素、血磷)與腎功能指標(估算腎小球過濾率estimated Glomerular Filtration Rate,以下簡稱eGFR,血中尿素氮及肌酸酐),並透過描述性統計、相關性分析及多元迴歸分析,探討慢性腎臟病患者營養狀態與腎功能惡化的相關性研究。
研究結果:結果顯示,血清白蛋白濃度與eGFR呈顯著正相關(p<0.001),表示白蛋白濃度較低者腎功能惡化風險較高;血磷濃度與eGFR呈負相關(p<0.001),表示血磷升高與腎功能下降密切相關;血紅素濃度亦與eGFR呈正相關(p<0.001),表示貧血與腎功能惡化有顯著關聯。此外,本研究發現糖尿病患者之eGFR顯著低於非糖尿病患者,指出糖尿病為影響腎功能惡化的重要共病因子。本研究確認血清白蛋白、血磷與血紅素可作為CKD患者腎功能變化的重要預測指標,強調臨床應加強營養監測與早期介入,以延緩腎功能惡化,並改善患者預後。
