Abstract
背景
B型肝炎病毒感染是世界上最常見的慢性病毒感染。慢性B型肝炎與原發性肝細胞癌的發展相關,其風險較非B型肝炎病毒帶原者的病人高出非常多,長期感染也可能造成病人會忍受肝炎發作時起伏波動的過程,將對肝臟造成不可逆的損害,病人容易發展為肝硬化,甚至肝功能不全、失代償性肝硬化和死亡。
目的
本臨床試驗設計的主要目的是要確認已長期接受核苷類藥物治療的慢性B型肝炎e抗原陽性病人使用試驗新藥ropeginterferon alfa-2b此長效型干擾素相較於真實世界中使用entecavir治療的病人的安全性及有療效評估設計。
方法
本試驗為一項開放性、單中心、活性藥物對照試驗,會篩選慢性B型肝炎且B型肝炎表面抗原陽性的病人在已穩定接受核苷(酸)類似物療程至少3年的受試者加入試驗,安排使用試驗藥ropeginterferon alfa-2b 450 μg;對照組由真實世界臺北醫學大學數據庫資料篩選慢性B型肝炎且B型肝炎表面抗原陽性的病人已穩定接受核苷 (酸)類似物治療3年後仍繼續服用Entecavir病人的臨床數據資料。當收到一位受試者時,會由數據庫分析數據資料篩選指定背景資料相同的病人作為對照組,以利分析成效。每位受試者將接受共48週的治療和24週的治療後回訪,本試驗的參與期間共72週,包含至少12次回訪,每次回診項目包括抽血檢查、理學檢查、肝功能相關檢查等,並且會記錄併用藥物及發生的不良反應。根據過去的臨床試驗研究結果,已知ropeginterferon alfa-2b較常發生的不良反應如下:嗜中性白血球減少、血小板數減少、白血球數減少、肌肉痠痛、頭痛、發燒、咳嗽、上呼吸道感染等。
結論
本臨床試驗的設計目的在於可以評估新藥ropeginterferon alfa-2b是否可助於B型肝炎e抗原轉陰性甚至表面抗原轉陰性並減緩或停止B型肝炎的惡化,期待此新藥能治癒更多患有B型肝炎的病人。
