Abstract
目的:本研究目的為探討中高齡者不同認知功能障礙程度的語言特徵表現。並以此建立語言特徵預測早期認知功能障礙的模型。
方法:研究對象為50歲以上具有不同認知功能程度的中高齡長者,由訪員在四間住宿型長照機構、二個社區失智據點、及一間醫院神經科進行面對面問卷訪談,並採用波士頓診斷失語症檢查圖片(Goodglass & Kaplan, 1983)在看圖說故事部分進行語音錄音資料收集,將語音檔轉為逐字稿後進行分析。最終收案人數扣除資料不全者後共129人進行分析。認知功能測量使用蒙特利爾認知評估量表(Montreal Cognitive Assessment, MoCA)評估。自變項包含語意、不完整特徵(僅名詞或動詞、缺少主詞、缺少受詞、缺少謂語、代名詞替代主詞)、詞性比例(名詞、動詞、形容詞、副詞、代名詞)、停頓、重複、贅句、總字數、總詞彙數、總句數等語言特徵。全樣本分為2/3建立語言預測模型及1/3驗證模型。統計方法採用描述性統計、雙變項分析和線性迴歸進行分析。
結果:總樣本的認知功能依正常、輕度、中重度障礙分的百分比為15.5%、41.1%、43.4%。建立模型並控制人口變項時,以語意得分越高(β=1.283)、形容詞比例越高(β=0.438)、贅句越少(β=-0.419)、缺少謂語越少(β=-2.334),與較好的認知功能表現顯著相關(R2=0.567)。以此語言模型用於認知功能分組時,區分正常、輕度障礙、中重度障礙三組時之正確分組比例達62.8%。若為在區分正常與障礙者時,正確分組比例為79.1%,AUC(0.936)較高,敏感度高但特異度較差;而在區分正常或輕度障礙相對於中重度障礙者時,正確分組比例達81.4%,以及可接受的AUC(0.847)、敏感度(0.667)、特異度(0.840)、與Kappa值(0.515)。由此顯示語言模型對於已有認知障礙者中,分辨輕度或中重度者的能力較佳。另外,當依照性別、教育程度進行分層分析,不同背景的參與者其與認知功能顯著相關的語言變項存在差異。
結論: 特定的語言特徵能用於預測認知功能障礙。本研究結果代表語言特徵可以作為辨別日常認知功能下降的指標,未來能應用於發展初步認知障礙篩檢語音工具。
