Abstract
肝癌(Hepatocellular carcinoma, HCC)是世界盛行的癌症之一,在全球癌症死亡排名為第三。它的發生率表現出極大的地理差異:北美與歐洲較低,非洲與亞洲較高。在台灣,肝癌是癌症發生率的第三位,其死亡率高居癌症死因的第一位。流行病學的研究指出肝癌的危險因子包括有年齡、B型肝炎病毒、HBV病毒量、C型肝炎病毒、黃麴毒素、肝硬化、代謝異常的疾病、男性賀爾蒙、喝酒及抽菸等。HBV與HCV為肝癌化的主要病原。但是肝纖維化與肝硬化確是肝癌發生中最關鍵的過程。肝癌發展初期的症狀並不明顯,現今的治療方式只對約30%的病人有效。因此早期診斷、早期治療能提高肝癌患者的存活率。在過去的幾十年的研究中指出,除了基因的改變外,腫瘤抑制基因因為啟動區的高度甲基化使其以附基因不活化的方式成為腫瘤發生的重要機制。在癌前腫瘤與晚期腫瘤可以觀察到啟動子序列異常的甲基化。異常甲基化及組蛋白修飾與轉錄活化子和抑制子的蛋白質複合體之間有密切的連結,受到影響的基因會改變mRNA轉錄的表現。啟動子序列中CpG島的高度甲基化會導致mRNA轉錄表現的下降,使腫瘤抑制基因與腫瘤相關基因發生靜默有關。在先期的研究中,實驗室前人將肝血管瘤非腫瘤組織的正常肝組織DNA pool、非肝腫瘤部分與成對的組織DNA pool以Illumina Infinium®甲基化平台的HumanMethylation27磁珠晶片分析,得到候選基因的資料。接著將此資料與肝癌甲基化基因專利前案之專利地圖比較,挑選出14個新穎的候選基因優先使用細胞株、肝癌病人與健康人的組織檢體,以MS-PCR 及 bisulfite sequencing方法進行驗證。本研究期望能夠藉此開發早期偵測肝癌之新穎性DNA甲基化生物標記。結果顯示,這些基因除了在肝癌細胞株中存在有高度甲基化的現象,在肝癌組織中也能偵測到甲基化現象;以單基因來說,89號候選基因的特異性最佳,16號候選基因敏感性最佳;多基因來說,16號候選基因與89號候選基因的組合之敏感性與特異性為最佳。因此得以推論本研究的4個候選基因是具有潛力可以作為肝癌的DNA甲基化生物標記。
