Abstract
前言:勃起功能障礙(Erectile Dysfunction, ED)影響著全球數百萬男性,往往嚴重降低生活品質。目前的治療方法,例如PDE5抑制劑和手術介入,主要提供短期症狀緩解,但無法解決ED的根本原因。因此,這些治療對於長期管理,尤其是針對複雜或重症患者而言,通常並不令人滿意。最近,幹細胞治療的進展為滿足這些未解需求帶來了新的希望。
幹細胞治療通過針對ED的根本原因而成為一種有前景的替代方案。幹細胞具有獨特的能力,可分化為與勃起功能相關的重要細胞類型,包括神經細胞、內皮細胞和平滑肌細胞。與傳統治療不同,幹細胞治療具有通過抗炎、促血管生成和抗細胞凋亡的作用實現真正的組織修復與再生的潛力。臨床前研究在多種ED模型中(包括糖尿病、神經損傷及與年齡相關的退化)展現了令人鼓舞的結果,表明幹細胞治療有可能在傳統方法不足的情況下提供持久的解決方案。
儘管臨床試驗仍然有限,但初步研究結果強調了幹細胞治療對於標準治療無效患者(包括糖尿病相關ED患者)的潛力。然而,進一步研究對於優化應用方案、評估長期安全性以及解決倫理問題至關重要。通過探索最佳的幹細胞類型和輸送方法,持續的研究可能幫助將ED治療從症狀緩解轉變為真正的再生療法,從而改善患者的治療效果與生活品質。
方法:在即將到來的一年中,我們計劃開展一項研究,探討糖尿病與高血壓對ED的影響,同時評估單劑量注射臍帶間質幹細胞(Umbilical Cord Mesenchymal Stem Cells, UC-MSCs)的安全性與療效。本開放性試驗旨在招募30名可評估參與者,並確保至少20名完成研究。治療將採用陰莖海綿體內注射方式,直接注射至陰莖海綿體中,並在注射前後使用評估量表進行評估。希望通過這項研究,我們能為細胞療法在治療勃起功能障礙中的作用提供有價值的臨床證據。
結果:此試驗於西元2024年12月4日經台北醫學大學暨附屬醫院聯合人體研究委員會審核核准進行 (案號:N202410052)。預計於西元2025年6月將本案送入衛生福利部食品藥物管理署進行案件審核。
