Abstract
敗血症是因感染引起的全身性發炎反應症候群(systemic inflammatory response syndrome, SIRS) ,會造成瀰漫性細胞和組織損傷,最終導致多重器官衰竭,有極高的致死率,世界衛生組織表示因敗血症導致的死亡幾乎佔全球死亡人數的20%。營養治療是敗血症病人臨床治療上一個重要的項目,然而現在對於高蛋白質給予對於病人預後影響尚不清楚,本研究目的是對於加護病房(Intensive care unit, ICU)敗血症病人給予目標熱量及高蛋白質飲食對其預後影響。本研究以台灣中部某地區醫院之ICU敗血症病人為對象,以病例回溯方式收集2021~2022年病人於ICU第一週內熱量、蛋白質攝取量,並根據病人實際熱量及蛋白質攝取達目標的程度,將病人分為熱量攝取<1100 kcal/day組和熱量攝取≥1100 kcal/day組,以及分為蛋白質攝取<1.2g/kg BW組及蛋白質攝取≥1.2 g/kg BW組,分析病人熱量和蛋白質於ICU入住期間白蛋白生化數值、7天死亡率、28天死亡率之影響。結果顯示熱量攝取≥1100 kcal/day可降低病人7天及28天死亡率,在高蛋白質(1.2g/kg BW/day)給予下,對加護病房敗血症之病人能降低加護病房7天死亡率,雖然在血漿白蛋白沒有顯著差異,但可以透過降低發炎相關指標,包含WBC、C反應蛋白(C-reactive protein,CRP)及前降鈣素原(procalcitonin,PCT),並可以降低重症疾病嚴重度SOFA的分數來改善加護病房敗血症病人的預後。以台灣中部某地區醫院加護病房中敗血症病人為研究對象,當病人飲食達目標熱量或蛋白質的攝取,有助於降低加護病房病人7天死亡率,且可降低WBC、CRP以及PCT,並使加護病房敗血症病人有較佳的預後。
