Abstract
背景: 常見精神障礙為盛行率高的心理疾患,受遺傳、環境及生物心理社會因素影響。近期研究顯示,DNA甲基化可能參與常見精神障礙的發生。與DNA甲基化、生物心理社會因素及心理健康的分子機制關聯仍不明確。此外,非藥物介入(如心理治療、認知行為治療、身體活動、正念及生物介入)廣泛應用於常見精神疾患照護,其調節DNA甲基化模式的潛力尚未確立。本論文整合生物(包含分子)及心理社會資料,提供常見精神障礙相關表觀遺傳輪廓的新穎洞見,特別強調可修飾因素。

目的: 調查DNA甲基化是否在生物心理社會決定因素與憂鬱症關聯中扮演調節角色,利用台灣人體生物資料庫的族群資料;並透過系統性回顧及薈萃分析,評估非藥物介入是否改變常見精神障礙個體的DNA甲基化水平。

方法: 採用多模態方法,包括族群統計分析(單變量、雙變量、多變量模型)及系統性回顧合併薈萃分析。第一階段為台灣人體生物資料庫2016–2017年資料的橫斷面分析(n=96,年齡19–74歲),探討生物、心理及社會變項與憂鬱症的交互作用,並檢視DNA甲基化的潛在調節能力。憂鬱症為自陳,統計模型包含條件邏輯斯迴歸。第二階段為全面系統性回顧及薈萃分析,匯總32篇合格出版品(16篇觀察性及16篇介入性)資料,評估非藥物方法(如認知行為治療、心理治療、生物介入、身體活動、正念)對DNA甲基化的影響。

結果: 第一階段顯示,身體質量指數升高與憂鬱症狀負相關(aOR=0.320,95% CI: 0.117–0.876,p=0.027),而IL6R甲基化水平較高則正相關(aOR=2.535,95% CI: 1.006–6.391,p=0.049),並對BMI–憂鬱症關係產生顯著調節效果(aOR=4.687,95% CI: 1.185–18.542,p=0.028)。第二階段顯示,非藥物介入顯著影響DNA甲基化。心理治療在所有介入類型中產生最強整體效果(SMD=0.75為高甲基化;SMD=–0.92為低甲基化)。在基因層面,認知行為治療下的NRN1基因展現最明顯甲基化變化(SMD=1.71),顯示其對認知基礎治療的高度表觀遺傳反應性。儘管依基因及介入類型方向(高甲基化 vs. 低甲基化)有所差異,整體異質性低至中度,支持這些發現的一致性。

結論: 本論文證實IL6R甲基化顯著調節代謝健康(BMI)與心理福祉的關係,突顯生物心理社會–表觀遺傳交互作用在憂鬱症中的角色。心理治療在介入類型中顯示最強整體影響DNA甲基化。在基因層面,NRN1展現最大雙向甲基化變化,特別在認知行為治療後,顯示其對認知基礎治療的高度表觀遺傳敏感性。透過整合分子、心理社會及介入資料,本研究貢獻精準精神醫學,並支持發展針對性非藥物策略用於憂鬱症預防與管理。未來研究建議實施前瞻性及介入性設計,以更明確評估表觀遺傳機制告知介入的因果角色及臨床效用。
