Abstract
研究背景:睡眠是人類維持健康的基本生理需求,研究已指出老年人經常會出現失眠或睡眠障礙的問題,當睡眠品質不佳時,會造成記憶力及注意力的衰退,出現焦慮、憂鬱等情緒問題,也會引起肥胖、糖尿病、高血壓及冠狀動脈心臟疾病,中風的風險也會上升,甚至增加死亡率。然而台灣現有的研究多針對某些地區或是居住於長照機構之長者的睡眠狀況進行調查,缺乏全國性的調查。
研究目的:探討台灣老年人自覺睡眠品質及其相關因素。
研究方法:使用衛生福利部國民健康署「中老年身心社會生活狀況長期追蹤調查」中的2015年新世代問卷進行次級資料分析,共有2,189位年滿65歲的長者,問卷內容包括人口學資料、身心功能狀況、健康狀況、社會支持及睡眠狀況。使用SPSS 18.0版套裝軟體,以平均值、標準差、次數、百分比、最小值、最大值、卡方檢定、皮爾森相關檢定、簡單邏輯斯迴歸及多元邏輯斯迴歸進行分析。
研究結果:研究發現台灣社區長者的睡眠潛伏期以少於15分鐘者居多,佔39.8%,實際睡眠時數達7.92(SD=1.61)小時,睡眠效率為
82.2%(SD=16.4%),另外,28.4%的長者自覺睡眠品質不好。研究也發現自覺睡眠品質的預測因子包括性別、日常生活活動功能、憂鬱症狀、孤寂感、睡眠潛伏期、實際睡眠時數及睡眠效率。
結論:本研究發現超過1/4的社區長者自覺睡眠品質差,其中,女性、憂鬱症狀較嚴重、較沒有孤寂感、睡眠潛伏期超過1小時、睡眠效率越低、和實際睡眠時數越少的老年人,自覺睡眠品質越不好。
應用及建議:研究結果可以提供醫護人員參考,除了疾病的照護外,也須評估社區長者的睡眠狀況,尤其是女性、有憂鬱情形、沒有孤寂感、睡眠潛伏期較長、睡眠效率較差及實際睡眠時數較少的老年人。另外,本研究結果也可以作為未來改善老年人睡眠品質相關研究的依據。
