Abstract
背景: 胰臟癌在2022年全球癌症發病率中排名第12,但死亡率卻高居第6位,是一種具有高度侵略性且預後不佳的惡性腫瘤。大多數胰臟癌為胰管腺癌,儘管全球的發病率和死亡率持續增加,但目前對於糖尿病與胰臟癌關係的研究主要依賴傳統統計方法,較少利用人工智慧和機器學習算法來開發更精確的預測模型。
目標: 本研究旨在利用廣泛的臨床數據,開發並驗證一種用於評估糖尿病患者罹患胰臟癌風險的機器學習預測模型,並識別與分析和風險增加相關的顯著預測因子。
方法: 本回顧性研究使用臺北醫學大學臨床研究資料庫,涵蓋三家醫院的資料,包括12個類別、71個數據表和2611個欄位。研究對象為被診斷為糖尿病且罹患胰臟癌的患者。我們使用單變量和多變量邏輯斯迴歸分析,並配合八種人工智慧演算法,包括邏輯斯迴歸(LR)、線性判別分析(LDA)、隨機森林(RF)、輕量梯度提升機(LightGBM)、梯度提升機(GBM)、極限梯度提升(XGB)、支持向量分類器(SVC)和投票集成(Voting),以開發預測模型。
結果: 多變量邏輯斯迴歸分析顯示,高血壓(OR = 2.4; 95% CI: 1.22-4.72)、使用利尿劑(OR = 3.12; 95%CI: 1.11-8.74)及更高的HbA1c水平(OR = 1.22; 95% CI: 1.04-1.42)顯著增加了胰臟癌的風險。在測試的機器學習演算法中,LDA表現最佳,其AUROC值為0.9073,超過了先前研究發表的結果,其中典型的AUROC值範圍在0.75至0.85之間。模型中影響最大的前五大臨床特徵包括:空腹血糖(Glucose AC)、糖化血色素(HbA1c)、高血脂症共病、抗糖尿病藥物使用以及降血脂藥物的使用。
結論: 開發的模型和研究結果為臨床醫生在管理和治療糖尿病患者時提供了重要的見解,有助於降低患者罹患胰臟癌的風險。本研究加深了我們對糖尿病與胰臟癌複雜關係的理解,展示了機器學習算法在臨床預測分析中的重要應用。
