Abstract
心血管疾病(CVD)是導致全球疾病負擔和死亡率的主要原因。準確的風險預測和早期干預能顯著改善患者的生存率。然而,目前大多數機器學習模型依賴於侵入性血液檢測指標和心電圖訊號進行預測,這限制其在資源有限的臨床環境(例如偏鄉、離島、中低所得國家)的應用。本文旨在全面評估十種機器學習模型的預測效能和運算效率,並探討僅使用非侵入性特徵,且不依賴高成本心電圖訊號進行模型訓練的可行性。本研究使用了一個多中心的心血管疾病資料集,包含462名患者記錄及11個臨床特徵。研究所使用的機器學習模型涵蓋五種經典機器學習模型──決策樹(Decision Tree)、隨機梯度下降(SGD)、多層感知器(MLP)、K-近鄰演算法(KNN)以及人工神經網絡(ANN);兩種深度學習模型──長短期記憶(LSTM)和卷積神經網絡(CNN);以及三種集成式學習模型──極端梯度提升(XGBoost)、輕量化梯度提升(LightGBM)和隨機森林(Random Forest)。本研究發現,在所提出的所有機器學習模型中,當使用11個特徵時,Random Forest的預測效能達到最高(AUC=0.8662),並且在僅使用九項(去除需抽血之侵入性特徵)或六項特徵(去除侵入性血液檢測指標及心電圖訊號)時,AUC分別為0.8634和0.8485,依然保持良好的競爭力及可用性。我們進一步透過SHAP(Shapley Additive Explanations)進行特徵重要性分析,並以表現最佳且具最高AUC的Random Forest作為解釋對象,發現「胸痛類型」、「最大心跳」和「運動誘發性心絞痛」是對模型預測貢獻最具影響力的前三項特徵。在運算效率方面,研究亦發現特徵維度的減少可顯著縮短模型的訓練時間。以Random Forest為例,當特徵數由11項減少至6項時,訓練時間由0.3524秒顯著下降至0.1819秒,顯示減少特徵有助於提升模型運算效率,相當適合在硬體運算資源有限的地區進行使用。綜上所述,本研究的實驗結果顯示基於非侵入性特徵、並排除心電圖訊號的Random Forest模型,在心血管疾病風險預測仍然具有良好的競爭力及可用性,並突顯該模型在資源有限的臨床環境(例如偏鄉、離島、中低所得國家)中實施的可行性,從而有機會將本研究所提出之模型推廣至這些地區,達成心血管疾病的早期檢測診斷和治療的目標。
