Abstract
背景:第1型糖尿病為需終身依賴胰島素治療的慢性疾病,青少年正值自我發展與責任轉移的關鍵時期,良好的自我管理對維持生理穩定與心理適應至關重要。本研究旨在探討第1型糖尿病青少年的自我管理能力與生活品質現況及其相關性,並分析其基本屬性與疾病特徵對自我管理及生活品質之影響。
方法:採橫斷式設計,於台灣北部某醫學中心兒童醫院內分泌科門診,以方便取樣招募100位診斷第1型糖尿病之青少年,透過結構式問卷收集資料。研究工具包含病歷、基本資料、「第1型糖尿病青少年糖尿病自我管理問卷」與「短版青少年糖尿病生活品質量表」。資料以 SPSS 26.0 統計軟體進行描述性統計、t檢定、單因子變異數分析、皮爾森相關與複迴歸分析。
結果: 共收案100位第1型糖尿病青少年,女性占60%,平均年齡為14.14歲(標準差2.91),罹病時間平均為5.95年(標準差3.99),平均糖化血色素為7.93%(標準差1.50),多數採指尖血糖監測(63%)及胰島素注射治療(97%)。自我管理面向分數最高為「目標」(平均值2.05,標準差0.93),最低為「糖尿病溝通」(平均值1.60,標準差0.99);生活品質面向分數最高為「對糖尿病的擔憂」(平均值4.35,標準差0.62),最低為「生活滿意度」(平均值3.85,標準差0.65)。整體自我管理與生活品質呈顯著正相關(r = 0.36,p < 0.05),其中「與父母合作」、「糖尿病問題解決」、「糖尿病溝通」及「目標」亦與生活品質達顯著正相關(all p < 0.05)。此外,性別、年齡、家庭社經地位及病友團體參與為自我管理之顯著影響變項。
結論:釐清影響第1型糖尿病青少年自我管理與生活品質之關鍵因素,顯示其兩者具有顯著正向關聯,且受性別、年齡、社經地位與病友團體參與等因素影響。可提供臨床照護與健康教育具體參考,建議醫療團隊強化青少年的目標導向、家庭合作與醫病溝通能力,並納入情緒支持,以促進其自我管理表現與整體生活品質。
