Abstract
咖啡因在興奮劑中扮演著相當重要的角色。咖啡因大部分都在小腸被吸收,攝取咖啡因數分鐘後,在血中就可以驗得到咖啡因的存在。其最大血中濃 度通常在 30 至 120 分鐘之間達到,並且有著接近百分之百的生體可用率。當咖 啡因被吸收後,並不會像大部分藥物一樣有著肝臟首度效應,因此咖啡因不管 是口服或是靜脈注射給予,其血中濃度差不多。基於上述原因,加上其容易取 得之特性,咖啡因在運動圈可算是運動員愛用之興奮劑之一。
然而,大部分關於咖啡因用於增強運動表現之研究資料蒐集多使用問卷調查,此研究方法相較於隨機分派試驗,較難分辨影響運動表現之原因是否單單來自咖啡因的攝取。嚴謹的隨機分派試驗相當必須。隨著越來越多的隨機分派試驗結果發表,也有不少系統性文獻回顧及統合分析被發表,然而收錄對象皆以男性居多。文獻指出,女性不管在運動表現或是咖啡因的吸收代謝和男性並不完全一樣。
本研究蒐集 15 篇研究品質佳之隨機分派研究,並以系統性文獻回顧及統合 分析之方法,探討女性運動員在運動前單次給予咖啡因,是否有增進無氧運動 表現之效果。
收錄研究之研究誤差使用 Cochrane's risk of bias tool 評估,研究方法評估使用的 工具為 Physiotherapy evidence database scale。評估結果發現,大部分研究在各 個誤差風險項目屬於低風險,研究方法皆屬於高品質。統合分析的結果顯示,在運動前 20~60 分鐘給予每公斤 3~6 毫克之咖啡因,在肌力 Muscle force (SMD = 0.27;95% CI=0.10∼0.44;P= 0.001;I2 =31.0%)、肌力(1RM) (SMD = 0.23;95% CI=0.12∼0.35;P< 0.0001;I2 =0%)、爆發力(跳躍高 度)(SMD = 0.24;95% CI=0.13∼0.35;P< 0.0001;I2 =31.0%)、爆發力 (Power)(SMD = 0.20;95% CI=0.13∼0.26;P< 0.00001;I2 =0.0%),以及 肌耐力 (Repetition to failure)(SMD = 0.30;95% CI=0.17∼0.43;P< 0.0001;I2 =0.0%)的運動表現皆有提升的效果。進一步排除有誤差風險疑慮之 研究後進行分析,結果同樣顯示,在無氧運動項目中,咖啡因能夠增進運動表 現。次族群分析中將肌群分為上、下半身,結果也同樣顯示,咖啡因對於下半 身肌肉之肌力與肌耐力依舊有增進的效果。
以現有證據看來,女性運動員若欲提升運動表現,咖啡因是一個相對安全且有效的選擇。
