Abstract
儘管末期腎臟病的治療已日趨完善,但末期腎臟病人依然有著極高的比率併發心血管疾病及免疫失調所造成之感染症;另一方面,有部分病人會出現紅血球生成素(Erythropoietin, EPO)反應不良,這些都大大影響了病人的生活品質甚至提高其死亡率,而有越來越多的研究指出這些問題與發炎息息相關。先前的研究發現Endothelin參與在心血管疾病、腎臟疾病及許多感染症的發生之中,並能刺激發炎前趨物質的釋放,做為引發發炎反應的介質。另一個與發炎相關的物質HLA-G,有強力的免疫抑制功能,除了與許多的自體免疫疾病、發炎性疾病有關,近年來也被發現也會影響到EPO在紅血球前趨細胞的作用。因此,本研究在第一部分將探討Endothelin家族 (EDN-1、EDN-2及EDN-3) 之基因多型性與末期腎臟病人中,發生需住院之嚴重心血管疾病、感染症,及EPO反應不良等併發症間的相關性;第二部分則會探討HLA-G基因多型性與EPO反應不良的關連性。
本研究從台北醫學大學附設醫院腎臟內科收納共190位透析病人,其中有86位曾因心血管疾病住院、91位曾因感染住院。另外利用紅血球生成素阻抗因子(EPO resistance index, ERI)之三分位數將病人分組後,Tertile 3共有60人,是為EPO反應不良組;Tertile 1共有61人,是為EPO反應良好組。
結果顯示,當EDN-1之rs5370帶有T/T基因型時或EDN-3之rs197174帶有G/G基因型,有較低的心血管疾病發生風險;而在EDN-3上的rs197173帶有G Allele時則有較高的心血管疾病發生風險。感染事件中,EDN-1的 rs2248580 帶有A/C基因型時或EDN-3的rs6064764帶有C allele時,會增加感染風險;EDN-2的rs11572340帶有A/A基因型時或EDN-3的rs926632帶有C allele時則會降低感染之風險。對紅血球生成素的反應中發現當EDN-1 之rs5370帶有T allele或EDN-3 之rs926632帶有T allele時,需要使用較高劑量的紅血球生成素以提升血色素;但當rs926632帶C/C時所需的劑量則較低。進一步分析EPO反應良好或不良時,發現EDN-1 rs3087459帶C/C或EDN-2 rs11572340帶A allele時有較高機率發生EPO反應不良,而當EDN-3之rs926632帶有C/C基因型則發生風險較低。另外,在HLA-G 3'UTR的兩個位點中皆未發現與EPO反應不良有相關性。本研究顯示,EDN的基因多型性可能藉由參與末期腎臟病病人中心血管疾病、感染的發生並影響了EPO之使用成效併發症的發生。
