Abstract
研究目的:探討性別差異下,老年人憂鬱症狀、失眠症狀與認知功能之相關性及檢視是否老年人失眠症狀為憂鬱症狀與認知功能障礙相關性之中介因子。
研究設計:本研究為次級資料庫分析,採橫斷式相關性研究。
研究方法:由2009年臺灣國民健康訪問調查(National Health Interview Survey)資料庫中,選取2175位65歲以上老年人,以單題調查主觀失眠症狀,包括困難入睡、困難維持睡眠、早醒。以短版美國流行病學研究中心憂鬱量表(Center for Epidemiologic Studies Depression Scale;CES-D scale)作為篩檢憂鬱症狀之工具與以簡易智能評估量表(Mini-mental state examination)測量認知功能。邏輯式迴歸分析與階層迴歸分析用以進行資料統計分析。
研究結果:共納入男性991位(45.6%)、女性1184位(54.4%);年齡範圍介於65-95.5歲。加入干擾因子進行控制後,男性老年人憂鬱症狀與認知功能障礙有顯著相關性(P = 0.02),但女性之憂鬱症狀與認知功能間相關性不顯著(P = 0.06);男性與女性老年人之憂鬱症狀與失眠症狀皆具有顯著相關(P皆<0.05);男性老年人困難入睡與認知功能障礙有關(P = 0.046),女性老年人則是困難維持睡眠與認知功能障礙有關(P = 0.01)。僅男性老年人困難入睡為憂鬱症狀與認知功能障礙之完全中介因子。
結論:本研究結果證實男性老年人之困難入睡為憂鬱症狀與認知功能障礙之完全中介因子。若能及早篩選老年人失眠症狀,進而介入適當護理措施,可能可進一步緩解男性老年人認知功能障礙。
