Abstract
細胞外囊泡(Extracellular Vesicles, EVs)已成為現代醫學中一種具潛力的組織再生與傷口修復治療方式。與傳統細胞治療相比,EVs 展現出多項優勢,例如使用安全性更高、免疫排斥反應較少、具備穿透組織屏障的能力,以及在儲存和運輸上的便利性。此外,EVs 的治療效果更穩定,使其成為備受關注的研究方向。研究顯示,利用幹細胞分泌的 EVs 可以有效減少發炎反應、促進血管生成、刺激細胞增殖,並加速傷口癒合。然而,儘管 EVs 在應用上展現出諸多潛力,仍面臨一些限制。例如,其生化組成尚未完全明確、缺乏標準化的生產與純化流程,以及目前缺少有效方法能穩定且大量促進 EVs 的釋放。特別是在促進血管生成的應用上,EVs 所含的血管內皮相關生長因子(如 VEGF、FGF 等)其表現量與活性常因來源細胞狀態與外在環境而有所變異,進而影響治療一致性與效果。因此,如何有效促進幹細胞所分泌的 EVs 中血管生成因子的表現,已成為推動其臨床應用的重要課題。
近年來,光生物調控(Photobiomodulation, PBM)被證實具備多種細胞調節功能,例如防止細胞凋亡、提升粒線體功能、促進三磷酸腺苷(ATP)與活性氧(Reactive Oxygen Species, ROS)生成,進而強化細胞增殖與修復能力。PBM 對幹細胞行為的影響逐漸受到重視,其中包括對 EVs 分泌量與內含物質組成的潛在調控能力。基於此,本研究嘗試利用 PBM 技術,透過特定波長的光來刺激脂肪衍生幹細胞(Adipose-Derived Stem Cells, ADSCs),不僅提升其活性並促進 EVs 的釋放,更進一步探討 PBM 是否能影響 EVs 中血管增生相關生長因子蛋白的表現差異。並透過多項實驗來進行驗證,包括細胞活性測試、細胞遷移實驗、蛋白質分析、細胞攝取及活性氧水平測定等,以深入探討 PBM 在特定雷射光照射下與 ADSCs 之間的相互作用機制,進一步闡明 EVs 釋放的潛在調控機制與應用前景。
研究結果顯示,PBM可顯著調節ADSCs分泌之EVs。經能量校準的近紅外光源(830 nm)在5 J/cm²劑量下能有效提升ADSC之活性、ATP含量與遷移能力,並在不引發過度氧化壓力的情況下促進ROS產生。PBM可使EV濃度提升,且其粒徑、電位與膜結構均保持穩定。PBM處理後的EV具更高的細胞活性促進效果與細胞攝取效率,顯示其在訊息傳遞與功能性上均有增強。此外,PBM亦可提升EV中血管新生相關蛋白VEGF的表達,進一步強化其促血管生成潛力。整體結果證實,PBM能同時提升ADSC功能與其EV品質與效率,為血管再生與慢性傷口癒合等再生醫學提供潛力。
