Abstract
目的:本研究探討了死亡顯著刺激效果對護理師道德傾向的影響,檢視死亡意識如何影響他們在傳染病控制及其他醫療情況中面對道德兩難困境時的倫理決策。
設計:本研究共有401位台灣的現職護理師參與,他們被分為死亡顯著刺激組與對照組。研究採用問卷評估參與者在個人及非個人情境中的道德傾向。分析了性別、年齡、工作年資、重症照護角色和醫療機構分類等因素。以道德判斷分數為依變項進行混合共變數分析(ANCOVA)。自變項包括醫院類型(醫學中心與其他)、單位類型(重症照護與一般照護)和死亡顯著刺激條件(MS與對照組),同時將性別和工作年資作為共變項。
結果:在死亡顯著刺激組和對照組中,參與者在面對非個人道德兩難困境時表現出更高的功利主義傾向(M = 0.73),相較於個人困境(M = 0.53),(F = 22.22, p < .001, η²p = .054),重複了先前研究結果。 醫院類型(醫學中心與其他)、單位類型(重症照護與一般照護)和死亡顯著刺激條件(MS與對照組)之間存在顯著的三向交互作用(F = 4.749, p = 0.030, η²p = 0.012)。由於醫院等級在病例複雜性和工作需求方面存在實質性差異,可能顯著影響護理師的倫理決策過程,因此對醫學中心和非醫學中心分別進行了事後分析。結果顯示,單位類型與死亡顯著刺激之間的二向交互作用僅在醫學中心顯著(F = 4.467, p = 0.036, η²p = 0.023),在非醫學中心則不顯著(F = 0.268, p = 0.605)。具體而言,在醫學中心工作的護理師,特別是在加護病房(ICU)和急診部門(ED)等重症照護角色中,在接觸死亡顯著刺激時表現出更高的功利主義傾向(M = 0.637, SE = 0.024),相較於對照條件(M = 0.608, SE = 0.027)。相反,一般照護護理師在接觸死亡顯著刺激時功利主義傾向顯著降低(M = 0.568, SE = 0.029),相較於對照條件(M = 0.657, SE = 0.033)。 這些發現表明,工作環境和角色顯著影響了死亡顯著刺激下的倫理決策。醫學中心的重症照護護理師似乎受死亡顯著刺激影響較小,顯示他們可能優先考慮公共利益而非個人權益。
結論:本研究強調了死亡顯著刺激對護理師道德傾向的差異性影響,這種影響基於其工作環境和角色。重症照護護理師表現出一致的功利主義傾向,這可能是因為他們經常接觸生死場景而形成的。應將處理道德兩難困境的教育課程納入護理教育,並提供針對性訓練,以支持在不同醫療環境中的倫理決策。
