Abstract
研究背景
策略訓練是一項旨在引導認知功能受損的中風患者生成個別化解決方案,並提升社會參與的復健治療。策略訓練過去已經被證實策略訓練介入組相較於控制組在社交參與頻率和生產力困難度呈現顯著改善,然而長期的效果尚未被證實。
研究方法
研究樣本包括來自多個中心的中風患者。該研究設計為一項前瞻性的隨機對照試驗,遵循評估者盲法原則,並將劑量匹配的反映性傾聽作為對照組,以評估策略訓練的有效性。研究共納入195名參與者,其中介入組有96名,控制組有99名,所有參與者均至少參加過一次介入訪談。本研究分析了5個時間點: 介入前、介入後、介入後三個月、介入後6個月與12個月的追蹤結果。採用社會參與量表(PM-3D4D)作為參與結果指標,急性後期活動測量(AM-PAC)作為活動指標,認知指標採用蒙特利爾認知評估(MoCA)和路徑描繪測試(TMT)與Stroop測試,並使用混合效應方法分析。
研究結果
PM-3D4D最終12個月的追蹤期(T5),實驗組展現出統計學上顯著的改善。分別在三個領域(社交、社區與生產性活動)當中的兩個向度(感知困難度與頻率)都有顯著的組別與時間交互作用,效應量整體介於小到中。功能性認知及日常活動領域有顯著改善。在認知評估中MoCA和TMT顯示與隨著時間有改善,但組間差異未達到統計學顯著性。僅在Stroop測試在基本處理速度(字色一致)方面顯示顯著的結果,在字色不一致下則沒有顯著。
結論
OPASS介入的優勢隨著時間的推移在各參與領域和維度中持續存在並更加明顯。日常活動和應用認知有所改善,而對於認知的測試則顯示出有限的變化。
