Abstract
研究背景
神經退化性疾病的盛行率在現今高齡化的社會逐漸升高,其中以帕金森氏症與阿茲海默氏症最為常見。藥物治療對其疾病症狀的控制十分重要,但過去文獻指出其用藥配合度與持續性不甚理想。本研究旨在探討神經退化性疾病病人對其疾病照護與用藥經驗、潛在用藥問題與影響用藥行為的因子,並評估藥事照護對此族群的影響。

研究方法
本研究共分為兩大部分:第一部分針對帕金森氏症門診病人進行半結構式質性訪談,探討病人及照護者對疾病治療的經驗與感受;第二部分採用前瞻性隨機對照試驗,納入輕至中度阿茲海默氏症病人與照護者,1:1分派至接受藥師衛教的實驗組或對照組,採取無母數與廣義估計方程式進行統計分析,探討對用藥持續性、配合度、生活品質、照護者負擔及失智症知能的影響,與影響其用藥行為的因子。

研究結果
第一部分納入15位病人及8位照護者。病人在確診後其與照護者皆需重新調適生活的步調;病人的用藥行為受服藥經驗、對副作用的認知、與醫病關係所影響,須重新規劃日常作息以減少藥效漸退的衝擊;良好的醫病互動能增加信任並提升用藥配合度,但有限的看診時間限縮了病人與醫師討論的機會;他們期望能對疾病與治療計畫有更多的了解,也期盼能找到治癒疾病的方式。第二部分實驗與對照組各納入20對病人及照護者,兩組皆有高度的用藥持續性(16/20 vs. 16/20)與配合度(87%–99%),未有顯著差異。實驗組照護者的用藥知能在藥師衛教後有顯著的提升(77.5 vs. 95.8, p < 0.01);照護者負擔的改變有對照組上升(+6.01; p = 0.07)而實驗組下降(-0.89; p = 0.78) 的趨勢,但未達統計上顯著差異。

結論
藥師對神經退化性疾病的病人及照護者進行藥事照護服務是重要且可行的,並能改善失智症照護者對疾病與治療的知能。未來研究可進一步探討藥師諮詢、用藥整合、與交互作用評估等全面的藥事服務對這類病人的用藥行為、生活品質與照護者負擔的影響。
